The Paramount Health Data Project (PHDP) is based on the knowledge that there is an interaction between health, poverty, and education that impacts overall health and wellness including academic performance.

Our Subscriptions

By implementing PHDP, you are able to leverage existing data and personnel in new and valuable ways to improve the academic achievement of your most vulnerable students and ensure that all of your student supports are working at optimum efficiency. 

  • This subscription activates the Paramount Health Data Project’s Academic Health Report Card which will be received at the beginning, middle, and end of the academic school year. This subscription also includes a weekly newsletter.

The monthly webinar will inform your team about including Health Data and your health professional/school nurse in your MTSS, 504 planning, and/or your IEP process.

We can also provide valid and reliable evaluation feedback on how well your academic and school-based health services are serving your students.

  • Our team of experts and researchers welcomes special projects related to health and education. If your school or community is navigating a public health crisis, a school-based health situation, or an educational issue with complex connections to health, we are here to help.

  • PHDP hosts an annual conference for school nurses, MTSS team members, and administrators, highlighting best practices, and emerging best practices specific to the opportunities present when health and education come together for the betterment of children. Topics include 504 planning, IEP development, and Medicaid billing practices.

  • We offer comprehensive training for paraprofessionals, specific to academic supports and healthcare.

  • We offer schools support as they navigate billing Medicaid for school-based health care.

  • PHDP & Anthem Blue Cross and Blue Shield are joining forces through a statewide collaboration to better understand the health problems common among Hoosier children navigating poverty that result in lower academic performance. By utilizing PHDP's data driven intervention model, schools and school based health care providers can tangibly work together to identify students who need support sooner to proactively address academic achievement based on health data. This work will provide greater context as to how health affects the ability to succeed in school and how educational outcomes directly affect future health outcomes. School based health solutions can provide primary care services, help address the social drivers of health, mental health care, social services, dentistry, and health education which can be critical to students’ education and long-term health. Additionally, these solutions would extend services to school staff, student family members, and others within the surrounding community. Sharing a goal to create a state in which everyone has a fair and just opportunity to be as healthy as possible and supporting the objectives of the State of Indiana School Based Health Center (SBHC) Initiative, we understand how much impact health can have on the educational advancement of our young people.
