Academic Health Report Card
The Paramount Health Data Project provides client schools with biannual reports summarizing student utilization of the school health center or school nurse and a summary of the relationship between academic performance and student health.
We help schools leverage large data sets to enhance and sustain existing academic and health support processes. We correlate academic achievement data and social determinants of health data for use in the MTSS process, 504 planning, and IEP development to ensure that professionals have valid and reliable data for faster on-the-ground decision-making.
Our report cards provide your team with comprehensive health data, physical and mental, that ensure that your school is fully leveraging and aligning all data sets for optimum student support. We help you define the Academic Health Cliff and how your health data can predict changes in student academic performance.

Evaluation of School Based Health Services
Let the experience and expertise of the PHDP team serve as your external evaluator for state, federal, or private grants focused on academic outcomes. We can also provide valid and reliable evaluation feedback on how well your academic and school-based health services are serving your students.

Annual Conference
PHDP hosts an annual conference for school nurses, MTSS team members, and administrators, highlighting best practices and emerging best practices specific to the opportunities present when health and education come together for the betterment of children. Topics include 504 planning, IEP development, and Medicaid billing practices.

Elevance Partnership
PHDP & Elevance Health are excited to announce a statewide partnership to understand better the health problems common among Hoosier children navigating poverty that result in lower academic performance. By utilizing PHDP's data-driven intervention model, schools and school-based healthcare providers can tangibly work together to proactively identify students who need support sooner to address academic achievement based on health data. This work will provide greater context as to how health affects the ability to succeed in school and how educational outcomes directly affect future health outcomes.
